Number Porting Letter of Authorization (LOA)

Request form for porting numbers to VOICE1 / NWSIP

Thank you for selecting VOICE1. Upon receipt of this form, we will start the transfer process and work with your existing carrier to transfer (port) your number(s). The porting process typically takes a few business days, but in extreme scenarios can take longer.   1. Once this form is submitted, the porting request cannot be canceled for ANY REASON.
  2. If you have a distinctive ring, a PIC freeze, or a carrier change restriction, you may need to remove it prior to porting your number(s). If you have a contract with your current provider that prohibits porting, you may not be able to port your number(s).
  3. The service address and name on this form must be the same as the records of your current provider, or your transfer request will be rejected or delayed.
  4. DO NOT contact your existing carrier to cancel service while we are attempting the transfer, or you may not be able to keep your telephone number/s. Only active numbers can be transferred. (To verify that your account has been canceled, contact the carrier five business days after the transfer completes.)
  5. We require a copy of your most recent phone bill and/or a screenshot of your account information. Most providers require verification of account information before allowing number porting. If porting numbers are from multiple accounts we will require documentation and a separate port order for each account.

Loosing Party Information

The following information must match your current providers information on record exactly. Loosing party may deny or reject the port if any information is not 100% accurate.

This is the name on file with your current (loosing) service provider. This much match exactly with their records or your port request will be denied.
Service Address(Required)
NOTE: this is not necessarily the same as your billing address. We require the location where the phone calls take place on file with the current operating network. If you have doubts about the location the operating network has on file, please request a customer service record (CSR). Only one service address is allowed per port order (if you have multiple locations, please submit multiple orders).
This is the losing provider where your numbers are to be ported away from.
If you have an account number please provide it here. Some providers require an account number to port numbers.
Some providers may have a Porting PIN that needs to be provided to port out a number. If your account has such a PIN please provider it here.
Some providers require the last 4 of the registered account owners' SSNs to port a number away. If your losing provider requires this provide it here.
Please enter a number from 0000 to 9999.

Number Porting Infromation

Please list each number to be ported or upload a file with the numbers to be ported. We will verify that the numbers listed are eligible to be ported. If they are not, we will notify you.
List the phone numbers to port as 11-digit numbers with no formatting. List one number per line.
Alternatively you can upload a list of numbers. once number per row, as either a .CSV, or .TXT document. If you encounter errors uploading please open a support ticket.
Accepted file types: txt, csv, Max. file size: 2 MB.
You must provide an invoice from the last 30 days with the phone number to be ported.

    Features to enable

    When porting the numbers over you may optionally enable the following features. Some features may have additional recurring fees.

    * Messaging requires 10DLC or Toll-Free Registry for any outbound A2P usage.
    Auto call forwarding
    If enabled, if your PBX is unreachable calls will be forwarded to the specified number.
    By signing below, I authorize VOICE1 LLC or its designated agent to act on my behalf and port the telephone number/s listed on this form to VOICE1 or its agent. I authorize the release of any information from my existing telephone provider to VOICE1 or its designated agent. I understand all fees associated with VOICE1 services and/or this transfer and understand that I may consult with VOICE1 at 1.855.252.8788 for such information. I am authorized to make these changes for the telephone number/s referenced above.
